Dietary Approaches for Reducing Chronic Disease Risk: Cardiovascular Health, Diabetes, Cancer, and Respiratory Wellness

In my previous article, Chronic Disease Prevention: What You Can Influence and What You Can't, I looked at what chronic disease means, the factors that influence risk, and why lifestyle is only one piece of a much larger picture.

This time, I want to get more specific about food.

No single diet prevents every chronic disease. No single food protects you from everything that could possibly go wrong.

What makes a difference is your overall eating pattern.

Research consistently shows that eating patterns emphasizing vegetables, fruits, legumes, whole grains, nuts, seafood or other nutritious protein sources, and unsaturated fats are associated with better health outcomes. These patterns also tend to be lower in excess sodium, added sugars, refined grains, and red and processed meats.

What foods reduce chronic disease risk?

Vegetables, fruits, whole grains, beans and other legumes, nuts, seeds, nutritious protein sources, and foods rich in unsaturated fats can all contribute to an eating pattern associated with lower chronic disease risk.

What eating pattern is best for preventing chronic disease?

There isn't one single best pattern. Mediterranean-style, DASH, and other eating patterns that emphasize minimally processed foods, plants, fiber, nutritious proteins, and unsaturated fats can support long-term health.

Can diet reduce the risk of heart disease?

Yes. Eating patterns rich in vegetables, fruits, whole grains, legumes, nuts, fish, and unsaturated fats, while limiting excess sodium, added sugars, and saturated fat, can help reduce cardiovascular disease risk.

What foods help reduce type 2 diabetes risk?

Fiber-rich foods such as vegetables, fruits, whole grains, beans, lentils, nuts, and seeds can support a lower risk of type 2 diabetes. Overall diet quality and eating pattern matter more than eliminating a specific food or nutrient.

Can diet reduce cancer risk?

Yes. Eating patterns rich in vegetables, fruits, whole grains, and legumes are associated with lower risk of some cancers. Limiting processed meat, excess alcohol, and highly processed foods can also be part of a cancer-prevention approach.

Does diet affect respiratory health?

Nutrition supports overall health and can be especially important for people living with chronic respiratory conditions. Adequate energy, protein, vitamins, minerals, and fluids can support the body, although no universal diet prevents or treats respiratory disease.

Is there one diet that prevents chronic disease?

No. There isn't one diet that prevents every chronic disease. The strongest evidence supports overall eating patterns that provide adequate nutrition, include a variety of foods, and emphasize vegetables, fruits, whole grains, legumes, nutritious proteins, and unsaturated fats.

And the good news is that many of the same habits support more than one area of health.

Cardiovascular Health

Heart health is one of the clearest examples of where overall eating patterns can have an influence.

A heart-supportive eating pattern generally includes:

  • Plenty of vegetables and fruits

  • Whole grains

  • Beans, peas, lentils, nuts, and seeds

  • Fish and other nutritious protein sources

  • Unsaturated fats, such as those found in olive oil, nuts, seeds, and fish

  • Less saturated fat

  • Less sodium

  • Less added sugar

  • Fewer highly processed foods

The American Heart Association's current dietary guidance emphasizes these same characteristics and specifically recommends replacing saturated fat with unsaturated fat. It also recommends choosing foods lower in sodium and added sugars.

Notice that it doesn't list “forbidden” foods.

It focuses on what people consume regularly in their overall eating pattern.

For example, adding beans to meals, choosing whole grains more often, eating a variety of vegetables and fruits, and using unsaturated oils more frequently can all contribute to a heart-supportive eating pattern.

Type 2 Diabetes Risk

Nutrition plays an important role in reducing the risk of type 2 diabetes, but no single carbohydrate, protein, or fat ratio works for everyone.

Current diabetes prevention guidance recognizes several eating patterns, including Mediterranean-style, plant-based, DASH, and lower-carbohydrate approaches. What they share is more important than their differences: overall diet quality, with an emphasis on foods such as whole grains, legumes, nuts, fruits, and vegetables, and less reliance on refined and highly processed foods.

Fiber is especially important here.

Beans, lentils, whole grains, vegetables, fruits, nuts, and seeds provide fiber along with other nutrients. Fiber can affect how quickly carbohydrates are digested and absorbed, and higher-fiber eating patterns are associated with lower type 2 diabetes risk.

And this is where I want to push back on some common nutrition advice.

You don't have to eliminate carbohydrates to manage blood glucose. Rice, potatoes, oats, beans, fruit, and other carbohydrate-containing foods can all fit into a nutritious eating pattern. The type of carbohydrate, amount, overall meal, and individual health needs all matter.

Cancer Risk Reduction

Food and cancer are an area where nutrition claims can get particularly exaggerated and even alarmist – as if what someone did or didn’t eat is to “blame” for someone getting cancer.

Another common thing is that you may see claims that a particular food, spice, vitamin, or antioxidant "fights cancer." That's usually much more complicated than the headlines or tropes suggest.

Stronger evidence supports overall dietary patterns than individual so-called cancer-fighting foods (or, conversely, cancer-causing foods).

The American Cancer Society recommends an eating pattern that includes a variety of vegetables, fruits, whole grains, and fiber-rich legumes, while limiting or avoiding sugar-sweetened beverages, highly processed foods, and refined grain products. It also recommends limiting red and processed meat.

Fiber-rich foods are also highly important, particularly because higher fiber intake is associated with lower colorectal cancer risk.

Alcohol also needs to be addressed. Alcohol is associated with increased risk for several cancers, and the American Cancer Society states that it is best not to drink alcohol for cancer prevention. For people who do choose to drink, limiting intake reduces, but does not eliminate, alcohol-related cancer risk.

This is an important example of why nutrition advice needs context. Something can be relatively small in one area of health/disease risk and much more significant in another.

Respiratory Wellness

Nutrition isn't a treatment for chronic respiratory diseases such as asthma or COPD, and eating a particular food won't prevent every respiratory problem.

But there is still an association here; nutrition still matters, particularly for people who already live with chronic lung disease.

Adequate energy, protein, vitamins, minerals, and fluids help support overall health and the muscles involved in breathing. For people with COPD, eating habits can also affect how comfortable meals feel and how much energy is available throughout the day. The American Lung Association recommends individualized nutrition guidance, adequate protein, and a varied eating pattern for people living with COPD.

For some people with COPD, large meals can make breathing more difficult because of abdominal pressure and breathing mechanics (the diaphragm, which is important for helping us breathe, is a muscle that is like the “dividing line " between the GI tract and the heart and lungs). Smaller meals, eating more slowly, and paying attention to meal timing may help.

And I want to make particular note of this: suggesting everyone with a respiratory condition needs a special "anti-inflammatory diet" is not accurate.

There is no universal respiratory diet.

Individual needs matter and must be considered.

One Eating Pattern Can Support Several Areas of Health

One of my favorite things about looking at dietary patterns instead of individual nutrients is that the same foods can support multiple areas of health.

Consider beans.

They provide protein, fiber, vitamins, and minerals. They can fit into a heart-supportive eating pattern, contribute to blood glucose management, and provide fiber associated with lower colorectal cancer risk.

Or consider whole grains.

They provide fiber and other nutrients and can fit into eating patterns associated with better cardiovascular and metabolic health.

Vegetables and fruits provide fiber, vitamins, minerals, and a wide range of other naturally occurring compounds.

Nuts and seeds provide unsaturated fats, protein, fiber, and other nutrients.

The goal isn't to find the one “magical” food that does everything.

Spoiler: because there isn’t one.

The goal is to build an overall eating pattern that does a lot of things reasonably well.

What About "Healthy Foods"?

This is where I think we can bring this around to real world nutrition, real world scenarios.

A healthy eating pattern doesn't require every meal to look nutritionally “perfect.”

It can include frozen vegetables, canned beans, whole-grain bread, rotisserie chicken, leftovers, restaurant meals, convenience foods, cultural foods, and foods chosen simply because they taste good.

We don’t have to optimize every bite.

We can create an eating pattern that provides adequate nutrition and works in your actual life.

And remember, this is key: food is only one part of chronic disease risk.

Physical activity, tobacco use, alcohol, sleep, genetics, environment, access to health care, medications, and other factors all matter, too.

The Bigger Picture

Dietary approaches for chronic disease prevention overlap a lot.

More vegetables and fruits. More fiber-rich foods. More whole grains. More legumes, nuts, and seeds.

Nutritious protein sources.

Unsaturated fats in place of more saturated fats.

Less excess sodium and added sugar. Less processed and red meat. Less alcohol, or none.

Those aren't rules for a particular diet.

They're characteristics that show up repeatedly in eating patterns associated with better health outcomes.

And there is still plenty of room for individual preferences, cultural foods, convenience, restaurants, celebrations, and pleasure.

The goal isn't to eat for disease prevention every minute of every day.

The goal is to develop an eating pattern that supports your health over time.

That's a much more realistic and appropriate way to think about nutrition and chronic disease risk.
